Responsibility:  Staff Member      


  1. The person observing the accident, or the first person informed of an accident by a student, parent, or volunteer must complete the top portion of a Student Accident Report Form (attached) before the end of shift and route to the nurse.   Bring student to the nurse or contact nurse for further guidance.  Do not hesitate to dial 911 if deemed necessary or unable to contact nurses when applicable.

  1. Complete the “Action Taken” portion of the Student Accident Report form and keep form in student’s chart.
  2. Inform staff members of issues or cause of accident when needed.

Responsibility:  Residential Life


  1. If an accident occurs in the cottage, the accident should also be fully docu­mented in the cottage shift report log and the Student Accident Report Form should be forwarded to the Health Center.
